• Provides professional nursing care, assessment and evaluations within scope of practice.
• Must have active LPN license in state of practice.

Responsibilities

• Performs licensed practical nursing functions under RN guidance.
• Completes resident assessments and plans, prioritizes care using nursing process.
• Administers medications safely and accurately per physician orders.
• Participates in care planning and updates plans as needed.
• Maintains accurate documentation and communicates care changes to physicians.
• Supports staff education and orientation for new team members.
• Investigates incidents and reports to leadership per policy.
