Care Plus Management at Lincoln Village Healthcare Center in Lincoln, IL is seeking a licensed practical nurse (LPN) to provide bedside care under RN supervision, administer medications, and support the care plan to promote resident health.

The role requires strong clinical skills and excellent communication, with leadership duties supervising CNAs and onboarding new staff. Full-time, 12-hour shifts (6-6) available, with benefits and a $10,000 sign-on bonus.

Qualifications

• Illinois LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Experience in long-term care or healthcare setting preferred.
• Strong clinical, communication, and leadership skills.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents' health status and report changes to RN/physician.
• Assist with activities of daily living (ADLs) and hydration.
• Document care, participate in care planning, and communicate with the care team.
• Supervise CNAs and assist in onboarding new staff.
