Job summary

Care Plus Management's Lincoln Village Healthcare Center in Lincoln, IL is seeking a Licensed Practical Nurse (LPN) to provide nursing care under the supervision of an RN and in line with facility standards. Qualifications

• Active Illinois LPN license in good standing.
• Graduation from an accredited practical nursing program.
• Prior experience in long-term care or similar healthcare setting preferred.
• Strong clinical skills and ability to respond quickly in emergency situations.
• Excellent communication, leadership, and organizational skills.

Responsibilities

• Administer medications and treatments as prescribed, in accordance with facility policies and state regulations.
• Monitor residents’ health status, observe for changes in condition, and promptly report findings to the RN or physician.
• Assist residents with activities of daily living (ADLs) as needed.
• Ensure residents receive appropriate nutritional support and hydration.
• Accurately document all care provided, medications administered, and resident observations in the medical record.
• Participate in care plan development and interdisciplinary team meetings.
• Communicate effectively with residents, families, and healthcare team members.
• Direct and monitor the work of CNAs to ensure quality care.
• Provide training, guidance, and feedback to CNAs as needed.
