The ideal candidate holds an active LPN/LVN license in the state, BLS certification, and has experience in long-term care. This position emphasizes patient safety, accurate documentation, and regulatory compliance.

Formación

• Graduate of an accredited practical or vocational nursing program.
• Prior experience in long-term care or skilled nursing setting preferred.
• LPN/LVN licensure in the state of practice is required.
• Basic Life Support (BLS) certification is required.

Responsabilidades

• Administer medications and treatments in accordance with physician orders and facility policy.
• Perform resident assessments and monitor for changes in physical, cognitive or behavioral status.
• Supervise and delegate appropriately to Certified Nursing Assistants on the assigned unit.
• Complete accurate and timely documentation in the resident record and medication administration record.
• Communicate changes in resident condition to the Registered Nurse, physician and family as appropriate.
• Follow infection control, safety and regulatory requirements governing long-term care practice.
