Job summary

Central Queens Rehabilitation and Nursing Center is seeking a Licensed Practical Nurse (LPN) to perform comprehensive nursing duties in a long-term care setting in New York. The role requires adherence to state regulations and DOH guidelines, with responsibilities including rounds, shift reports, documentation in Sigma EMR, and coordination of care with physicians and staff.

The ideal candidate will have a NY LPN license and 1 year of long-term care experience, plus familiarity with Sigma EMR.

Qualifications

• 1 year of long-term care experience preferred
• Current NY LPN license to practice
• Knowledge of Sigma EMR is a plus
• Ability to make independent decisions when needed
• Strong clinical documentation skills

Responsibilities

• Determines quality of care by interpreting facility policies and state regulations; enforces adherence to nursing acts and DOH regulations.
• Completes rounds to ensure provision of resident care across all areas during the shift.
• Provides shift change reports to CNA staff and maintains shift-to-shift documentation.
• Maintains Sigma dashboard and ensures documentation of medication and treatments by end of shift.
• Ensures documentation for transfers, appointments, and dialysis in Sigma.
• Maintains good customer relations by communicating with residents and families and answering questions.
