Job summary

Central Queens Rehabilitation and Nursing Center is seeking an licensed practical nurse (LPN) to provide quality resident care in a long-term care setting. You will perform rounds, document in Sigma EMR, monitor vitals, administer medications, and communicate with RNs and families.

Requirements include NY LPN license, 1 year of long-term care experience preferred, ability to make independent decisions, and strong documentation skills.

Qualifications

• 1 year of experience in long-term care preferred.
• Currently licensed LPN in NY.
• Ability to make independent decisions when needed.
• Professional interaction with residents, families and staff.
• Strong documentation and clinical skills.

Responsibilities

• Performs rounds and ensures quality resident care.
• Completes narcotic counts and records in Sigma.
• Maintains accurate progress notes and daily reports.
• Documents medication administration and treatments in Sigma.
• Communicates changes in resident condition to RN/charge nurse.
