Job summary

WeCare Centers is seeking Licensed Practical Nurses (LPN) for as-needed shifts at The Green Home Care Center in Wellsboro, PA. Nurses will monitor residents, administer meds, assist with daily activities, and collaborate with the healthcare team to ensure quality care.

Qualifications

• Active LPN license in PA is required.
• Experience in long-term care or skilled nursing is preferred.
• Strong documentation and record-keeping skills.

Responsibilities

• Conduct comprehensive assessments of patients upon admission and during stay.
• Provide regular patient status updates to the care team.
• Assist with implementing treatment plans under RN supervision.
• Complete required nursing documentation accurately and timely.
• Administer medications and supervise other staff as needed.
• Provide direct resident care as required.
• Adhere to planned care and maintain care standards.
• Report to the Director of Nursing regarding patient status.

Job Duties

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.

• Provide regular patient status updates.

• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.

• Complete required documentation in an accurate and timely manner.

• Supervise, administer and or assist with medications.

• Provide direct care of residents as necessary.

• Follow the plan of care for each resident.

• Ensure the residents care standards are maintained.

• Report to the Director of Nursing.

Qualifications
