Job summary

WeCare Centers is seeking Licensed Practical Nurses (LPNs) for full-time night shifts at Avalon Springs Care Center, 745 Greenville Road, Mercer, PA 16137. The LPN will provide essential nursing care, monitor health, administer medications, support daily activities, and collaborate with the healthcare team to ensure quality resident care.

Qualifications

• LPN license in PA required.

Responsibilities

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout treatment.
• Provide regular patient status updates.
• Assist in implementing patient’s treatment plan under RN direction.
• Complete required documentation accurately and timely.
• Supervise, administer and/or assist with medications.
• Provide direct care of residents as necessary.
• Follow the plan of care for each resident.
• Report to the Director of Nursing.

Job Duties

• Supervise, administer and or assist with medications.

• Ensure the residents care standards are maintained.


• Report to the Director of Nursing.
