Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Private Duty LPN to provide in-home, patient-centered care in Memphis, TN. You will work part-time with both day and overnight shifts, ensuring dignity and independence for clients in their homes.

The role offers flexible scheduling with opportunities for growth within a growing company, backed by a supportive team and resources from Adoration Health.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license.
• Valid driver's license and proof of insurance.
• CPR certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 care for the client according to the plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions, communication with family and care team.
• Coordinate or order supplies, medications, and meals as needed.
• Ensure all patient needs are met per physician instructions.

Schedule: Part-Time; Days & Overnight (2 nurses)

• Adult Case
• Trach/Vent/G-tube experience required
• Saturday & Sunday 7:00am - 7:00pm
• Tuesday & Thursday 7:00pm - 7:00am

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
