Adoration Health is seeking a Private Duty Licensed Practical Nurse for the Collierville, TN area to provide high-quality, in-home care under a client-specific plan. You will administer medications, perform ostomy care, and monitor patient status while coordinating with families and the case manager.

This full-time position offers benefits, 1:1 caregiving, and opportunities for professional growth within a supportive home-health team.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice.
• Valid driver’s license and proof of car insurance.
• CPR professional certification.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence.
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results.
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs.
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand.
• Ensuring all patient needs are being met in accordance with the instructions of the physician.

Office Location: Cordova, TN

Coverage area: Collierville, TN

Schedule: Full-Time, Overnight (2 nurses)

• Adult Case
• Trach/G-Tube experience preferred
• Monday - Sunday 6:00am - 6:00pm
