Job summary

Adoration Health is seeking a Private Duty Licensed Practical Nurse to join our Memphis, TN team. You will deliver 1:1 patient care, administer medications, monitor progress, and coordinate with the care team to ensure quality outcomes.

The role requires an LPN/LPT/LVN license, CPR certification, and 1+ years of nursing experience, preferably in home health. Valid driver’s license and reliable transportation are required. Full-time overnight schedule with growth opportunities.

Qualifications

• LPN/LPT/LVN degree with current, unrestricted state license.
• CPR certification is required.
• At least 1 year of nursing experience, preferably in home health.

Responsibilities

• Provide 1:1 client care according to the care plan while preserving dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed and assess outcomes.
• Document nursing actions and communicate with family, case manager, and physicians; report changes to the care team.
• Coordinate or order supplies, medications, and feedings to ensure adequate stock.
• Ensure patient needs are met per physician instructions.

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
