Job summary

Adoration Health is seeking a Private Duty LPN to join our Columbia, TN team. You will provide patient-centered 1:1 care in a private duty setting, following each client's plan of care while preserving dignity and independence.

Responsibilities include administering medications, managing feedings and ostomy care, and documenting progress for the care team. The role offers 24/7 coverage with day and overnight shifts and opportunities for advancement.

Qualifications

• LPN/LPT/LVN license in state of practice with active registration.
• CPR certification required.
• 1+ year nursing experience preferred.
• Home health care experience preferred.
• Valid driver's license and proof of insurance.

• Provide 1:1 client care following the care plan.
• Administer medications, feedings, oxygen, ostomy care per plan.

Schedule: Full-Time; Days & Overnight (24/7 care; 4 nurses needed)

• Pediatric Case
• G-tube experience preferred
• 7:00am - 7:00pm

Responsibilities

As a Private Duty Licensed Practical Nurse, you will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
