Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Private Duty Licensed Practical Nurse to provide patient-centered 1:1 home care in Columbia, TN. The role includes medication administration, documentation, and coordination with families and clinicians.

You will work full-time days and overnight shifts in a supportive team, delivering high-quality care in a home setting with opportunities for growth and stability.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in the state of practice
• Valid driver’s license with proof of car insurance
• CPR certification
• 1+ years nursing experience preferred
• Home Health care experience preferred

Responsibilities

• Providing 1:1 care per client’s plan while preserving dignity and independence
• Administering medications, feedings, oxygen, ostomy care and assessments
• Documenting nursing actions and communicating with family and physicians
• Coordinating or ordering supplies, medications, and feedings as needed
• Ensuring patient needs are met per physician instructions

Schedule: Full-Time; Days & Overnight (24/7 care; 4 nurses needed)

• Pediatric Case
• G-tube experience preferred

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
