Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Private Duty Licensed Practical Nurse in Bell Buckle, TN. This full-time role offers days and overnight shifts, 1:1 patient care, and opportunities to work independently with team support.

You will administer meds and care per the plan, document progress, and coordinate supplies while ensuring dignity and safety for patients at home.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice.
• Valid driver’s license with acceptable driving record and proof of insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 client care according to the plan of care, preserving dignity and independence.
• Administer medications, feedings, oxygen, ostomy care per plan and document actions and outcomes.
• Coordinate or order supplies, medications, and feedings to ensure adequate stock.
• Ensure patient needs are met in accordance with physician instructions.

• Pediatric Case
• G-tube experience preferred
• Friday & Sunday
• 7:00am - 11:00am
• 7:00am - 3:00pm
• 3:00pm - 11:00pm

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of resultsAccurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
