Adoration Health is seeking a dedicated Private Duty LPN to join our team in Bell Buckle, TN. You will provide 1:1 patient care in home health settings, following each client's plan of care and ensuring dignity and independence.

Responsibilities include administering medications, monitoring progress, documenting nursing actions, and coordinating supplies. Full-time days and nights with flexible scheduling options, competitive benefits, and opportunities for growth.

Qualifications

• LPN/LPT/LVN license in the state of practice.
• Valid driver’s license and proof of insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred; Home Health care experience preferred.

Responsibilities

• Providing 1:1 care for client per plan of care while maintaining dignity and independence.
• Administering medications, feedings, oxygen, ostomy care as needed per plan of care and assessments.
• Document nursing actions and communicate with family, case manager, and physicians; inform changes in condition.
• Coordinating or ordering supplies, medications, and feedings to ensure adequate on-hand quantities.
• Ensuring all patient needs are met per physician instructions.

• Pediatric Case
• G-tube experience preferred
• 7:00am -11:00pm
• 7:00am - 3:00pm

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferredHome Health care experience preferred
