Job summary

Adoration Health in Afton, TN seeks a Private Duty LPN to provide 1:1 patient care in the home setting. You will administer medications, assist with feeding, oxygen, and ostomy care, and document all nursing actions with families and physicians.

You will coordinate supplies and communicate progress to the case manager, ensuring patient needs align with the plan of care and physician instructions, while maintaining dignity and independence.

Qualifications

• LPN/LPT/LVN license in the state of practice.
• CPR professional certification is required.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 care for the client according to the plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed and assess results.
• Document nursing actions and communicate with family, case manager, and physicians; report changes in condition.
• Coordinate or order supplies, medications, and feedings to ensure adequate on-hand quantities.
• Ensure all patient needs are met per physician’s instructions.

Schedule: Full-Time; Days & Overnight

• Pediatric Case
• Vent/Trach/G-tube experience preferred
• Day Shift: 7:00am - 7:00pm, 3 nights per week
• Overnight Shift: 7:00pm - 7:00am, 3 nights per week

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
