Job summary

Adoration Health is seeking a Private Duty Licensed Practical Nurse to join our team in Woodbury, TN. The role involves providing 1:1 patient care per the individual plan of care, including medications, feeding, oxygen, and ostomy care, with careful documentation and family communication.

The Private Duty LPN works in a home-health setting with a Murfreesboro, TN office location and Woodbury coverage, offering a full-time daytime schedule (Mon-Fri, 9:00am–4:00pm) and strong team support along

Qualifications

• LPN/LPT/LVN license in TN
• CPR certification
• 1+ year nursing experience preferred
• Home health care experience preferred

Responsibilities

• Provide 1:1 client care following the plan of care while preserving dignity and independence
• Administer medications, feedings, oxygen, ostomy care as needed and assess results
• Document nursing actions and communicate with family, case manager, and physicians
• Coordinate or order supplies, medications, and feedings to ensure availability
• Ensure patient needs are met per physician instructions

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
