Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Private Duty LPN to join our Woodbury, TN team with primary location in Murfreesboro, TN. You will provide dedicated 1:1 patient care in home settings and follow a client-specific care plan.

Qualified candidates hold an LPN/LPT/LVN license in Tennessee, CPR certification, and 1+ year nursing experience; home health experience is preferred. Benefits include medical/dental/vision, 401(k), PTO, mileage reimbursement, and more.

• Valid driver’s license with acceptable driving record.

Responsibilities

• Providing 1:1 care per the client’s plan of care.
• Administering medications, feedings, oxygen, ostomy care as needed.
• Documenting nursing actions and communicating changes to care team and family.
• Coordinating supplies and equipment as needed to ensure on-hand stock.
• Ensuring patient needs are met per physician instructions.

Schedule: Full-Time; Days

• G-tube experience preferred

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
