Job summary

Adoration Health in Greeneville, TN is seeking a Private Duty LPN to provide 1:1 patient care in the home setting. The role offers part-time daytime hours with a supportive team and flexible scheduling options.

You will administer medications, document care activities, and coordinate with families and case managers to ensure quality, compassionate care while maintaining patient dignity.

Qualifications

• LPN/LPT/LVN license in state of practice.
• Valid driver's license with proof of insurance.
• CPR certification (professional).
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 client care following the plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate or order supplies, medications, and feedings as needed.
• Ensure all patient needs are met per physician instructions.

• Adult Case
• Skilled nursing
• Sunday, Monday Tuesday 9:00am - 5:00pm
• Wednesday 9:00am - 3:00pm

Responsibilities

As a Private Duty Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
