Adoration Health seeks a Private Duty LPN to join our Greenback, TN team. You will provide 1:1 patient care in home settings, administer medications, and document progress in coordination with family and physicians.

This role offers a full-time daytime schedule and opportunities for growth within a supportive, patient-centered environment. The ideal candidate has an LPN/LPT/LVN license, CPR certification, and at least 1 year of nursing experience, with home health experience preferred.

Qualifications

• LPN/LPT/LVN degree from an accredited college with current license in the state.
• CPR professional certification is required.
• 1+ years of nursing experience preferred; home health experience preferred.

Responsibilities

• Provide 1:1 nursing care following the plan of care while preserving dignity.
• Administer medications, feedings, oxygen, ostomy care as needed per plan.
• Document nursing actions, results, and changes for physicians and families.
• Coordinate supplies, medications, and feedings to ensure availability.
• Meet patient needs per physician instructions.

• Pediatic Case
• G-tube experience preferred
• Monday - Friday 7:00am - 7:00pm

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
