Haven Hospice in Orange Park, FL is seeking a dedicated Licensed Practical Nurse (LPN) for full-time field-based care. You will provide 1:1 hands-on comfort, follow care plans, document thoroughly, and educate patients and families about hospice services.

Competenze

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the State.
• Must be a licensed driver with insured vehicle and CPR certification as required.
• Demonstrate good verbal and written communication, and organization skills.

Schedule: FT ON CALL- Monday - Thursday 5pm to 3am, on call 3am to 8am.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the State.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.
• Understand philosophy of hospice concept. Also understand needs of the terminally ill.
• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
• Demonstrate good verbal and written communication, and organization skills.
