Job summary

Haven Hospice is seeking a dedicated Hospice LPN to join our field team in Orange Park, FL. You will provide patient-centered end-of-life care under state nursing guidelines, following care plans and documenting care with empathy and professionalism.

This role offers field-based work with on-call shifts evenings and nights, opportunities for professional growth, and a supportive team environment focused on compassionate care for patients and families.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the State of Florida.
• Must have a valid driver’s license with insured vehicle.
• Maintain CPR certification where required.

Responsibilities

• Provide hands-on 1:1 patient care in the field.
• Follow care plans and document care accurately.
• Educate patients and families about illness and care plan.
• Coordinate care with hospice team (aides, chaplains, social workers).
• Manage pain and symptoms; monitor treatment effectiveness.
• Participate in training and quality improvement activities.

As a Hospice LPN You will:

• Give hands-on care to patients nearing the end of life, following state nursing rules.
• Follow the care plan after assessing the patient’s needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team (like aides, chaplains , social workers).
• Manage pain and symptoms, checking how well treatments are working.
• Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the State.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.
• Understand philosophy of hospice concept. Also understand needs of the terminally ill.
• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
• Demonstrate good verbal and written communication, and organization skills.
