Job summary

Hospice of the Midwest in Guthrie Center, IA is seeking a passionate LPN to join our team. You will provide patient-centered end-of-life care following state nursing rules and individualized care plans.

This full-time role offers a stable schedule (Mon-Fri 8am-5pm), mileage reimbursement, and opportunities for growth within a supportive team. Travel within the Guthrie territory is required to serve patients in their homes.

Qualifications

  • Graduate of an accredited practical nurse or vocational nursing program.
  • Minimum of three years nursing experience; hospice or medical/surgical preferred.
  • Currently licensed as an LPN/LVN in the state.
  • CPR certification required.
  • Must be a licensed driver with insured vehicle.
  • Understand hospice philosophy and end-of-life care.

Responsibilities

  • Provide hands-on care to patients nearing end of life following the care plan.
  • Keep clear and timely records of care provided.
  • Teach and support patients and families about illness and care plan.
  • Coordinate care with hospice team including aides, chaplains, and social workers.
  • Manage pain and symptoms and monitor treatment effectiveness.
  • Participate in training and quality improvement activities.

As a Hospice LPN You will:
  • Give hands-on care to patients nearing the end of life, following state nursing rules .
  • Follow the care plan after assessing the patient’s needs.
  • Keep clear and timely records of the care provided.
  • Teach and support patients and their families about the illness and care plan.
  • Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).
  • Manage pain and symptoms, checking how well treatments are working .
  • Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
  • Currently licensed as an LPN/LVN in the State.
  • Comply with accepted professional standards and practice.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Possess and maintain current CPR certification.
  • Understand philosophy of hospice concept . Also understand needs of the terminally ill.
  • Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
  • Demonstrate good verbal and written communication, and organization skills.
