Job summary

Hospice of the Midwest is seeking a dedicated LPN to provide compassionate, patient-centered care in a hospice setting. The role requires thoughtful assessment, adherence to care plans, and clear documentation while supporting families through difficult times.

The LPN will work within a supportive team, enjoy a stable full-time schedule, and contribute to ongoing quality improvement efforts in a growing organization.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the state.
• Comply with professional standards and practice.
• Must be a licensed driver with insured vehicle.
• Maintain CPR certification.
• Understand hospice philosophy and needs of the terminally ill.
• Use knowledge and experience to make decisions and solve problems.
• Demonstrate good verbal and written communication and organization skills.

Responsibilities

• Give hands-on care to patients nearing end of life, following state nursing rules.
• Follow the care plan after assessing the patient's needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team.
• Manage pain and symptoms, checking how well treatments are working.
• Take part in training and quality improvement activities to grow professionally.

As a Hospice LPN You will:

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the State.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Possess and maintain current CPR certification.
• Understand philosophy of hospice concept. Also understand needs of the terminally ill.
• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
• Demonstrate good verbal and written communication, and organization skills.
