AMEDISYS HOME HEALTH is seeking a licensed practical nurse to provide in-home patient care across assigned visits. You will verify plans of care with an RN, collaborate with physicians, educate patients and caregivers, monitor vital signs, and perform medication reconciliation as part of comprehensive home-based nursing support.

Applicants must have an active LPN license in the relevant state, CPR certification, and a valid driver’s license with reliable transportation.

Formación

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsabilidades

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform procedures ordered by physician (e.g., Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
