Job summary

Amedisys in Gray, GA seeks a licensed practical nurse (LPN) to join our home health team. You will verify plans of care with RNs, collaborate with physicians and RNs, teach patients self-care, monitor vitals, reconcile medications, and perform physician-ordered procedures. Timely documentation is essential.

Full-time days with competitive pay and a $5,000 sign-on bonus plus mileage reimbursement. Comprehensive benefits available.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state-specific where assigned.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs; notify when condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform procedures ordered by physician (e.g. Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
