Job summary

Adoration Home Health in Williamsburg, VA is seeking a licensed practical nurse to join our team as a Home Health LPN. You will provide patient-centered, 1:1 care in the home setting, following the plan of care and maintaining the client’s dignity.

This full-time role requires an active LPN/LPT/LVN license, CPR certification, a valid driver’s license, and 1+ year of nursing experience. Home health experience is preferred. Competitive hourly pay and a supportive environment.

Qualifications

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and state of residence.
• Valid driver’s license, acceptable driving record and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home Health care experience preferred.

Responsibilities

• Providing 1:1 care for the client following the client’s plan of care at all times while maintaining the client’s dignity and independence.
• Administering medications, feedings, oxygen, ostomy care as needed per client’s plan of care and providing assessments of results.
• Accurately documenting nursing actions, communication with family, case manager, and physicians, and documenting progress and outcomes for established goals; informing physicians, case managers, and families of changes in the client’s medical condition and needs.
• Coordinating or ordering supplies, medications, and feedings as needed to ensure adequate amounts are available.
• Ensuring all patient needs are met in accordance with the physician’s instructions.
