Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health Licensed Practical Nurse (LPN) to join our team in Bluefield, WV. This role provides patient-centered, 1:1 care in the home setting with a supportive team and a focus on dignity and independence.

You will administer medications, document actions, coordinate supplies, and ensure patient needs align with the physician plan. A valid WV license, CPR, and driver’s license are required.

Qualifications

• LPN/LPT/LVN degree and current unrestricted license in the state of practice and residence (WV).
• CPR certification is required.
• Valid driver's license with proof of insurance.

Responsibilities

• Providing 1:1 client care following the plan of care while maintaining dignity and independence.
• Administering medications, feedings, oxygen, ostomy care per plan and assessments.
• Documenting nursing actions and communicating with family, case manager, physicians; update progress and inform changes in condition.
• Coordinating or ordering supplies, medications, and feedings to ensure adequate stock.
• Ensuring all patient needs are met per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
