Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Home Health LPN to serve Elizabethtown, NC. You will provide patient-focused, 1:1 home health care with a supportive team available as needed.

Schedule: Monday–Friday, 8a–5p, with opportunities for advancement in a growing organization. Benefits include comprehensive health plans, retirement savings, and wellness discounts.

Qualifications

• LPN/LPT/LVN degree from an accredited nursing program with valid license in state of practice and residence.
• Valid driver’s license, driving record acceptable and proof of insurance.
• CPR certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 care per the client’s plan of care while maintaining dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care per plan and assessments of results.
• Document nursing actions, communications with family, case manager, and physicians, and track progress toward goals.
• Coordinate or order supplies, medications, and feeds to ensure adequate stock.
• Ensure patient needs are met per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
