Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health Licensed Practical Nurse to join our Fayetteville, WV team. Provide 1:1 in-home care based on the patient’s plan of care, administering medications and therapies while maintaining dignity and independence.

You will document actions, communicate with families and clinicians, and coordinate supplies. This full-time role offers stability and opportunities in a growing home health network.

Qualifications

• LPN/LPT/LVN degree from an accredited college with active license in state of practice and residence.
• Valid driver’s license and proof of car insurance.
• CPR certification.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 home health care following the plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate ordering supplies, medications, and meals as needed.
• Ensure patient needs are met per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
