Job summary

Adoration Health in Van Buren, AR seeks a dedicated LPN for Home Health. You will provide patient-centered, 1:1 care during a standard 8a-5p schedule, supporting patients in their homes with safety and compassion.

Responsibilities include administering meds and care per plan, documenting actions, and coordinating supplies while maintaining communication with families and physicians. Home health experience is preferred.

Qualifications

• LPN/LPT/LVN license in good standing in the state of practice.
• Current unrestricted registration in the state of residence.
• Valid driver's license and proof of car insurance.
• CPR professional certification.
• 1+ years nursing experience; home health preferred.

Responsibilities

• Provide 1:1 care per the client's plan of care while maintaining dignity.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate changes to physicians and families.
• Coordinate supplies, medications, and equipment to ensure availability.
• Ensure patient needs are met per physician's instructions.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
