Job summary

Amedisys in Tennessee is seeking a licensed practical nurse for a part-time, benefits-eligible role delivering home health care to patients in their homes.

You will verify plans of care with RNs, collaborate with physicians, teach patients self-care, check vital signs, reconcile medications, perform procedures as ordered, and document timely and accurately to support high-quality care.

Competitive pay range listed: $22.50–$26.25 per hour, plus comprehensive benefits and opportunities for growth.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
