Job summary

Abound Home Health Care seeks a reliable Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. The LPN works under the supervision of an RN and follows each patient’s individualized plan of care.

The role includes monitoring vital signs, administering medications, wound care, and patient education, with documentation of visits and communication of changes to the supervising RN.

Qualifications

• Active LPN license in Indiana.
• Experience in home health or community-based care preferred.
• Ability to monitor vital signs and changes in patient condition.
• Ability to administer medications and treatments accurately.
• Wound care skills as applicable to home health settings.
• Provide patient and caregiver education.
• Document visits timely and accurately.
