Job summary

Adoration Health is seeking a Home Health LPN to join our Jasper, AL team. Provide 1:1 patient care in home settings, administer meds and treatments, and document progress with families and physicians. This full-time role supports independence while offering team backup and growth opportunities.

The position emphasizes compassion, thorough documentation, and adherence to each client’s plan of care, with a service-oriented environment across Jefferson county and surrounding areas.

Qualifications

• LPN/LPT/LVN degree with active license in state of practice and residence.
• CPR certification or equivalent professional CPR required.
• Valid driver's license with acceptable driving record and proof of insurance.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Providing 1:1 home health nursing care following the patient’s plan of care.
• Administering medications, feedings, oxygen, ostomy care as needed.
• Documenting nursing actions and communicating with family, case manager and physicians.
• Coordinating supplies, medications, and equipment to ensure availability.
• Ensuring patient needs are met per physician instructions.

Education

LPN/LPT/LVN Degree

CPR professional certification

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home H ealth care experience preferred
