Job summary

Adoration Health in Chattanooga, TN seeks a licensed practical nurse (LPN) for full-time home health care. You will provide direct 1:1 patient care per the plan of care, manage medications and therapies, and document all nursing actions with families and care teams.

The role emphasizes independence with team support, a stable company, and opportunities for advancement with a growing home health team.

Qualifications

• LPN/LPT/LVN degree from an accredited nursing program with current license in state of practice and residence.
• Valid driver's license and proof of car insurance.
• CPR certification (professional).
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 care for clients per their plan of care while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed per plan and assessments.
• Document nursing actions and communicate with family, case manager, and physicians; update progress and outcomes.
• Coordinate or order supplies, medications, and feedings to ensure adequate stock.
• Ensure all patient needs are met per physician instructions.

Schedule: Full-Time, Mon-Fri, 8AM-5PM

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home H ealth care experience preferred
