Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health LPN to join our Chattanooga, TN team. This full-time role offers 1:1 patient care, medication administration, and care coordination in a home health setting.

The LPN/LPT/LVN degree with current Tennessee license and CPR certification is required, with 1+ year of nursing experience preferred. Drivers license and reliable vehicle are needed for local travel, with supportive team and growth opportunities.

Qualifications

• 1+ years of nursing experience preferred.
• CPR professional certification.
• Valid driver's license, acceptable driving record and proof of car insurance.

Responsibilities

• Providing 1:1 home health care following the plan of care and maintaining patient dignity.
• Administering medications, feeding, oxygen, ostomy care, per plan and documenting results.
• Documenting nursing actions and communicating with family, case manager, and physicians.
• Coordinating or ordering supplies, medications, and feedings as needed to ensure supplies are on hand.
• Ensuring patient needs are met in accordance with physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
