Job summary

Avenues Recovery Center in South Bend, IN seeks a licensed practical nurse for the day shift at our detox and residential facility. You will provide direct patient care, perform assessments, monitor vitals, and support the care team.

This role requires Indiana LPN license or compact license, and at least 1 year in treatment settings. We offer comprehensive benefits and a collaborative, supportive workplace.

Qualifications

• Current, valid LPN/LVN license in Indiana or compact license with multistate privileges.
• 1 year experience in treatment industry preferred.
• Strong clinical assessment and documentation skills.
• Excellent teamwork and communication abilities.

Responsibilities

• Conduct comprehensive nursing assessments on admission.
• Perform routine shift duties: screenings, vitals, scheduling labs, updating charts.
• Address pharmacy needs, authorize prescriptions, dispense meds, maintain logs.
• Collaborate with medical, clinical and UR teams to coordinate care.
• Maintain accurate medical documentation and medication records.

for our Detox and Residential treatment facility in South Bend, IN

What You'll Do

• Complete comprehensive medical assessment for each client upon admission
• Perform all nursing shift duties (screening, vitals, scheduling appts and labs, updating charts...)
• Address pharmacy needs and prescription authorizations; dispense meds and maintain med logs
• Collaborate with medical, clinical and UR teams to ensure seamless coordination of care
• Ensure impeccable medical documentation, chart review, and medication logging for each client

What We're Looking For

• Current, valid licensure to practice nursing in-state, or compact license with multistate privileges
• 1 year experience in treatment industry preferred
• Strong critical thinking, oral and written communication skills
• Excellent collaborative, multitasking and time management skills
