Job summary

Avenues Recovery Center is seeking a Licensed Practical Nurse for the day shift at our detox and residential facility in South Bend, Indiana. You’ll conduct admissions assessments, monitor vitals, manage medications, and document care to support a seamless patient experience.

We value compassionate professionals with strong communication and teamwork skills. An in-state LPN license (or compact) and about 1 year in behavioral health help you thrive within our collaborative, patient-centered

Qualifications

• Current, valid LPN licensure in-state or compact license.
• 1 year experience in treatment/behavioral health preferred.
• Strong communication and teamwork skills.
• Excellent multitasking and time management.

Responsibilities

• Perform nursing shift duties: admissions assessments, vitals, scheduling and labs.
• Address medication needs and documentation; dispense meds and maintain logs.
• Collaborate with medical, clinical and UR teams to coordinate care.
• Ensure accurate medical records and chart reviews for each client.

Now Hiring LPNs for the Day Shift for our Detox and Residential treatment facility in South Bend, IN!

What You’ll Do

• Complete comprehensive medical assessment for each client upon admission
• Perform all nursing shift duties (screening, vitals, scheduling appts and labs, updating charts...)
• Address pharmacy needs and prescription authorizations; dispense meds and maintain med logs
• Collaborate with medical, clinical and UR teams to ensure seamless coordination of care
• Ensure impeccable medical documentation, chart review, and medication logging for each client

What We’re Looking For

• Current, valid licensure to practice nursing in-state, or compact license with multistate privileges
• 1 year experience in treatment industry preferred
• Strong critical thinking, oral and written communication skills
• Excellent collaborative, multitasking and time management skills
