Job summary

Elite Care - Oatfield Estates in Milwaukie, OR is seeking a passionate LPN to join our care team. You will promote residents' health, participate in care planning, and educate staff on appropriate care aligned with state and federal regulations.

The role requires an active Oregon LPN license and at least 1 year of LPN experience, with shifts Thursday-Monday or Tue-Sat, 3 p.m.–11 p.m.; weekend availability is required. Competitive hourly pay plus benefits offered.

Qualifications

• Must hold an active Oregon LPN license.
• Minimum 1 year LPN experience in related field/setting.
• Possess and maintain strong knowledge of Oregon Board of Nursing requirements, especially as they pertain to Nursing Delegation.
• Possess and maintain a thorough knowledge of Med Room components and Medication Administration.
• Able to pass an Oregon background check.
• Must be a U.S. citizen and reside within Oregon to apply.

Responsibilities

• Develop and support a system that allows for a constant flow of communication to the Director of Nursing regarding resident, family, or employee concerns, a decline in resident condition, employee injuries, budget disparities, or any matters that potentially impact operations
• Review and perform evaluations of resident health and functional care needs before moving in to ensure that the community can meet the resident’s needs and that regulatory requirements have been met
• Review all resident information and complete an assessment of the resident before move-in, when changes in the resident's condition occur, and as required by company policy and state regulation
• Assume on-call responsibilities on a rotational basis and assign on-call responsibilities to other qualified management
• Develop knowledge of community policies, procedures, and state/federal regulations pertaining to resident care
• Perform selected clinical tasks that reflect an unusual or unique condition in a resident or that cannot be delegated, i.e. IM injection
• Monitor the status of resident’s health condition and initiate actions in response to changing needs
• Assist in coordinating resident transfers, to or from a hospital or another health facility

Job Overview:

As a Licensed Practical Nurse with us, you'll be responsible for promoting the health and well-being of each resident by providing accurate nursing assessments, giving appropriate input to the development of care plans, and educating staff on the appropriate and proper care of each resident. You will also help to develop systems, procedures, and training programs that support governing state and federal regulations.

Compensation Range:

$40.00 to $45.00 per hour (based on experience)

Shifts:

Thursday-Monday, 3 p.m.–11 p.m. or Tuesday-Saturday, 8 hour shift. Shift days are flexible, but weekend availability is required.
