Prairie Vista Village is seeking a dedicated LPN charge nurse to join our team in Altoona, IA. The LPN will plan, organize, and assist in delivering direct and indirect resident care following established policies, nursing practice standards, and care principles.

Responsibilities include rounds, care plan development and evaluation, and supervision of nursing staff. The role requires CPR certification, state licensure, and prior long-term care experience is preferred.

Qualifications

• Must be a graduate from an accredited school and currently licensed as LPN in the applicable state.
• CEU requirements must be maintained.
• CPR certification preferred.

Responsibilities

• Assess and provide resident care following nursing standards and policies.
• Make daily rounds to monitor condition and comfort of residents.
• Assist with developing and evaluating care plans.
• Supervise nursing staff and ensure competency.

Role:

We are seeking a dedicated LPN - Licensed Practical Nurse to join our team! The LPN Charge Nurse plans, organizes, and assists in providing direct and indirect resident care following established policies and procedures, standards of nursing practice and principles of nursing care. Orients, instructs and supervises personnel.

Responsibilities:

• Assess and implement interventions for resident care based on standards of nursing practice and principles of nursing care, in accordance with Federal, State, and Company requirements.
• Make daily rounds to see all residents to ascertain condition and comfort.
• Initiate procedures to protect residents from injury and infection.
• Assist DON and/or RN with developing, implementing, and evaluating plans of care to include, but not be limited to, recognition of spiritual, emotional, psychosocial, and functional factors.
• Encourage and assist with restorative care in self-care and group activities.
• Monitor nursing personnel compliance with nursing policies and procedures and competency of clinical skills, evaluating nursing staff job performance with direction from DON.
• Communicate with the Physician as indicated by resident needs or status and assure follow-through; maintain hand-off communications with oncoming shift regarding resident care needs.
• Oversee direct care staff and ensures staff complete tasks and cares as assigned.
• Speak, read and write the English language in an understandable manner.
• Ability to apply principles of basic general math.

Qualifications:

• The LPN Charge Nurse must be a graduate from an accredited school, be currently licensed in the applicable state without encumbrance, be responsible for maintaining professional licensure and CEU requirements.
• Preferably 1-2 years of previous nursing experience as a Charge Nurse in Long Term Care and/or a Skilled Nursing Facility.
• Must be currently CPR certified.
