Job summary

Pivotal Health Care's Westchester Village in Lenexa, KS seeks an LPN to join our team. The LPN Charge Nurse will plan, organize, and assist in providing direct and indirect resident care, following established policies and standards, while orienting and supervising staff.

We offer 12-hour shifts with options for full-time, part-time, or PRN roles. Candidates must be licensed, CPR certified, and prepared to contribute to a compassionate, patient‑centered care environment.

Qualifications

• Graduate from an accredited school and be licensed in the applicable state without encumbrance.
• Preferably 1–2 years of nursing experience as a Charge Nurse in long-term care or skilled nursing facility.
• CPR certification is required.

Responsibilities

• Assess and implement interventions for resident care according to standards and requirements.
• Make daily rounds to monitor condition and comfort of residents.
• Oversee nursing staff and supervise competency and task completion.
• Communicate with physicians and ensure proper hand-off between shifts.

Role: We are seeking a dedicated LPN – Licensed Practical Nurse to join our team! The LPN Charge Nurse plans, organizes, and assists in providing direct and indirect resident care following established policies and procedures, standards of nursing practice and principles of nursing care. Orients, instructs and supervises personnel.

Responsibilities

• Assess and implement interventions for resident care based on standards of nursing practice and principles of nursing care, in accordance with Federal, State, and Company requirements.
• Make daily rounds to see all residents to ascertain condition and comfort.
• Initiate procedures to protect residents from injury and infection.
• Assist the Director of Nursing and/or RN with developing, implementing, and evaluating plans of care to include, but not be limited to, recognition of spiritual, emotional, psychosocial, and functional factors.
• Encourage and assist with restorative care in self‑care and group activities.
• Monitor nursing personnel compliance with nursing policies and procedures and competency of clinical skills, evaluating nursing staff job performance with direction from the DON.
• Communicate with the physician as indicated by resident needs or status and assure follow‑through; maintain hand‑off communications with oncoming shift regarding resident care needs.
• Oversee direct care staff and ensure staff complete tasks and cares as assigned.
• Speak, read and write the English language in an understandable manner.
• Apply principles of basic general math.

Qualifications

• Graduate from an accredited school and be currently licensed in the applicable state without encumbrance, responsible for maintaining professional licensure and CEU requirements.
• Preferably 1–2 years of previous nursing experience as a Charge Nurse in Long‑Term Care and/or a Skilled Nursing Facility.
• Must be currently CPR certified.
