The Grand Rehabilitation and Nursing at Queens is seeking a full-time Licensed Practical Nurse (LPN) to join our skilled nursing facility team in New York. The role focuses on geriatric care, medication administration (including IVs), vital signs, and resident education, with active collaboration with physicians and CNAs.

Immediate hiring and both full-time and part-time shifts available. Shift options include 7am-3pm and 3pm-11pm, with a competitive hourly rate and benefits package, including

Qualifications

• Current LPN license in NY.
• Ability to evaluate and treat independently without direct supervision.
• Experience providing care in a geriatric setting is preferred.

Responsibilities

• Provide care for residents in SNF/LTC units.
• Administer medications, including IVs.
• Take and record vital signs.
• Provide resident education and family education.
• Communicate with physicians, NPs, families and other healthcare professionals.

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
