Charter Senior Living of Woodbridge is seeking an LPN to support residents in our assisted living and memory care programs. We offer full-time and part-time shifts, including every other weekend, with a compassionate team focused on enhancing residents’ quality of life.

Qualifications

• Licensed practical nurse with an active LPN license in Connecticut.
• Experience in assisted living or memory care settings preferred.
• Ability to perform assessments, care planning, and documentation.

Responsibilities

• Assist residents in the Assisted Living/Memory Care programs.
• Manage medications and treatments per plan of care.
• Document nursing services and support teamwork with staff.

LPN/Licensed Practical Nurse POSITION SUMMARY:

A LPN/Licensed Practical Nurse is one of the main support providers to our residents. The position aids residents in the Assisted Living and/or Memory Care Program(s) through assessments and care planning, medication and treatment management, implementation, and evaluation of resident plans of care, documentation of nursing services and enabling residents to lead a quality life while maintaining their dignity. The position will enhance the lives of older adults by assisting them with patience and a friendly smile all while embracing teamwork and collaboration among all staff.
