Job summary

Bridge Senior Living in Birmingham, AL is seeking a committed Licensed Practical Nurse (LPN) for day shift to deliver and coordinate resident care in collaboration with physicians and the wellness team.

You will provide direct patient care, perform rounds, document care, and support development of resident care plans, with emphasis on compassionate service and a positive living environment for seniors.

Qualifications

• LPN license in applicable state.
• First Aid & CPR certification.
• At least 1 year of related work experience.

Responsibilities

• Assess and monitor residents’ health status.
• Coordinate care with physicians and care team.
• Develop and update resident care plans.
• Maintain communication with residents, families, and physicians.

Licensed Practical Nurse ( LPN) Job Summary:

Licensed Practical Nurse ( LPN) will implement and coordinate the delivery of care in collaboration with physicians and care associates. Above all, you will exhibit a passion for serving seniors and contributing to the positive environment our residents are proud to call home.

• Assessment, planning, implementation, and evaluation of our residents
• Provide high level of care by making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents’ overall condition and behavior.
• Assist the Director of Wellness in the development and update of resident care plans.
• Maintain ongoing communication with residents, families, and physicians regarding the resident’s care.
