Job summary

Springfield Healthcare Center is seeking Licensed Practical Nurses (LPNs) to join a compassionate, patient-centered team dedicated to delivering care with dignity and compassion. Qualifications

• LPN licensure/certification required.
• Strong clinical foundation and patient care skills.
• Ability to lead and coordinate care with CNAs and CMTs.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
