Accela Post Acute Care at Hamilton is seeking an LPN licensed Practical Nurse to join our on-site Hamilton, NJ team. You will provide compassionate care, administer medications, and support individualized care plans in a 55-bed post-acute setting on the RWJ University Hospital-Hamilton campus.

The role requires NJ-licensed LPNs with acute care LTC experience; CPR is preferred. Expect rotating weekends, a welcoming team, and ongoing training with mentorship.

Qualifications

• LPN license valid in New Jersey.
• Experience in acute care LTC or similar settings preferred.
• CPR certification is preferred.
• Strong verbal and written English communication skills.
• Professionalism and ability to maintain confidentiality.

Responsibilities

• Provide compassionate and personalized nursing care to residents.
• Collaborate with the healthcare team to implement care plans.

Weekend Requirement:

This position requires rotating weekends (on/off schedule). All Per Diem staff must commit to working at least one full weekend per month (both Saturday and Sunday).
