Job summary

Accela Post Acute Care at Hamilton is hiring a Licensed Practical Nurse (LPN) to provide compassionate, resident-centered care in a 55-bed post-acute setting on RWJ University Hospital-Hamilton campus.

You will work with a multidisciplinary team to implement individualized care plans, administer medications, monitor health, and educate residents and families. Strong English communication and confidentiality are essential; weekend rotation may apply.

Qualifications

• Current LPN license in New Jersey.
• Experience in Acute care LTC or similar settings.
• CPR certification preferred.
• Clear verbal and written English communication.
• Professionalism and confidentiality maintained.
• Ability to collaborate across all levels and teams.
• Customer service orientation and composure under pressure.

Responsibilities

• Provide compassionate, personalized care to residents.

Weekend Requirement: This position requires rotating weekends (on/off schedule). All Per Diem staff must commit to working at least one full weekend per month (both Saturday and Sunday).
