Job summary

Rockville Center is seeking a licensed practical nurse (LPN) to join our skilled nursing facility team. The ideal candidate will provide comprehensive care to residents, administer medications (including IVs where applicable), record vital signs, and collaborate with physicians, NPs, and families.

Shift is 7AM-3PM with every-other-weekend requirements. Eligible applicants must hold an active NY LPN license and be capable of independent evaluation and treatment with minimal supervision.

Qualifications

• Current LPN license in NY required.
• Weekend availability is required (alternate weekends).

Responsibilities

• Provide care for residents of SNF and Long Term Care Unit.
• Administer medications and monitor vital signs.
• Coordinate with physicians and other health professionals.
• Supervise CNAs and educate residents and families.

Rockville Center is searching for a talented and motivated Licensed Practical Nurse (LPN) to join our dynamic team.

The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of NY, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience.

LPN (Licensed Practical Nurse) Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

LPN (Licensed Practical Nurse) QUALIFICATIONS/REQUIREMENTS:

• Current License-LPN
