The ideal LPN candidate will:

• Be a graduate of an approved school of nursing
• Have an active LPN license in the State of New York
• Hold CPR certification
• Have the ability to observe and identify residents' needs, report to the RN/Supervisor, and collaborate with the RN in making clinical decisions
• Prioritize and coordinate activities of the nursing unit
• Be an excellent problem solver with strong decision-making skills
• Demonstrate leadership abilities
• Prior experience in a skilled nursing facility is preferred but not required
• Experience with SigmaCare is preferred but not required

About the position:

The LPN functions under the supervision of the RN/charge nurse and the Nursing Supervisor. Responsibilities include, but are not limited to:

• Leading and coordinating resident care in collaboration with CNAs, physicians, and other disciplines, including assessment and family involvement
• Planning and implementing care, documenting thoroughly in the EMR
• Proficiency or ongoing training in wound care, ostomy care, Foley catheter care, medication administration (including G-tube and IVs), trach care, and suctioning
• Monitoring for changes in condition and responding promptly to prevent re-hospitalizations and emergencies, responding efficiently and calmly during emergency situations
