Job summary

Milford Health & Rehabilitation is an affiliate of National Health Care Associates and a Great Place to Work certified facility in Milford, CT. We are seeking a Licensed Practical Nurse (LPN) for part-time shifts to provide direct nursing care and medication administration to our residents.

Open shifts include 7a-3p every other Saturday or Sunday, 3p-11p every other weekend, and 11p-7a (16 hours). You will work with CNAs and the healthcare team to implement care plans and ensure resident comfort.

Qualifications

• Valid state license (LPN)
• Compassionate and empathetic approach to patient care
• Strong nursing skills with ability to care for aged and chronically ill patients

Responsibilities

• Administer medications and treatments as prescribed
• Monitor vital signs and document medical information
• Collaborate with interdisciplinary team to develop care plans
• Provide wound care and other nursing interventions
• Supervise and support CNAs in delivering care
• Ensure compliance with regulatory standards and protocols

What You'll Do:

As a Licensed Practical Nurse, you will play a pivotal role in ensuring the well-being and comfort of our residents. You will provide direct nursing care, administer medications, collaborate with healthcare professionals, and oversee the implementation of care plans.

Key Responsibilities:

• Administering medications and treatments as prescribed by healthcare providers
• Monitoring and recording patient vital signs and medical information
• Collaborating with the interdisciplinary team to develop and implement care plans
• Providing wound care and other nursing interventions as needed
• Supervising and supporting Certified Nurse Assistants (CNAs) in delivering care
• Ensuring compliance with all regulatory standards and protocols
